CRATER SPORTS ELIGIBILITY
It is necessary to be on “Blue Card” status to play a fall, winter, or spring sport. This tells the coaches that the athlete is clear to practice sports.The athlete may not practice/participate until this paperwork is cleared at the athletic office.
To compete it must be determined that the athlete meets the academic requirements of the school, district, and state (O.S.A.A.) as stated in the Code of Conduct.
To receive blue card status
the athlete must present the following completed and signed forms to the Crater Athletic Office.
1. Code of Conduct – Done each school year before sport practice begins
2. Physical/medical history form – (double sided)
Physical – good for 2 years – OSAA FORM ONLY
Medical History – Done each school year before sport practice begins
3. Emergency-Medical-Insurance form – Done each school year before sport practice begins
4. A copy of the family insurance card – Done each school year before sport practice begins(Valid proof that athlete is insured) School insurance is available. Football insurance is separate from other sports.
Parents are responsible for reporting medical or insurance changes to the school
